Ensuring rapid post-discharge follow-up
A stroke patient was discharged from the hospital and recommended to Mosaic Pharmacy Service for post-discharge pharmacy care. The patient scheduled a follow-up appointment with a primary care physician 30 days post-hospital discharge. During our clinical review, Mosaic identified this gap between discharge and follow-up as inappropriate due to the patient’s diagnosis.
Mosaic engaged the patient’s caregiver to explain why an immediate appointment was needed and advised on language to help obtain an earlier follow-up appointment. A Mosaic team member followed up the next day and was told the patient had an appointment to see the doctor within a few days.
